
Our Transition Program in Residential Care is designed to support older adults as they move from hospital stays to more permanent living arrangements, whether that be returning home with added support or transitioning into long-term residential care. This short-term program, typically lasting up to 12 weeks, offers comprehensive services including nursing care, physiotherapy, occupational therapy, and social work, all aimed at helping individuals regain their independence and confidence after hospitalization
Upon enrollment, each participant receives a personalized care plan developed in collaboration with our multidisciplinary team. This plan addresses medical needs, daily living activities, and personal goals, ensuring a holistic approach to recovery. Our dedicated staff, including registered nurses, therapists, and social workers, provide continuous support, monitor progress, and adjust care plans as needed to facilitate a smooth transition.
Beyond medical and therapeutic services, the program emphasizes social engagement and emotional well-being. Residents are encouraged to participate in group activities, fostering connections with peers and reducing feelings of isolation.
